Missed a GLP-1 Dose? What the Labels Say
Last updated October 20, 2026
Short answer: It depends on the product. For the weekly injections, Ozempic allows a late dose within 5 days, Mounjaro and Zepbound within 4 days (96 hours), and Wegovy as long as your next scheduled dose is more than 2 days away. Past those windows, skip the missed dose and take the next one on your regular day. If unsure, call your prescriber or pharmacist.
Missed dose rules by medication
This is what each prescribing label says. These instructions apply to the FDA-approved products, not to compounded versions, which may come with different directions.
| Medication | If you miss a weekly dose |
|---|---|
| Ozempic (semaglutide) | Take it as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip it and take the next dose on your regular day. |
| Wegovy injection (semaglutide) | If your next scheduled dose is more than 2 days (48 hours) away, take the missed dose as soon as possible. If it is less than 2 days away, skip the missed dose and resume on your regular day. |
| Mounjaro (tirzepatide) | Take it as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip it and take the next dose on your regular day. |
| Zepbound (tirzepatide) | Take it as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip it and take the next dose on your regular day. |
| Wegovy tablets (daily) | Skip the missed dose and take the next dose the following day. |
In every weekly case, you then go back to your regular once-weekly schedule.
What if you miss more than one dose?
The Wegovy label addresses this directly: if you miss 2 or more consecutive doses of Wegovy injection, the dose escalation is restarted at a lower dose to reduce the risk of stomach side effects, and the patient information tells you to contact your healthcare provider about how to restart. The Ozempic, Mounjaro and Zepbound labels do not give a separate rule for several missed doses, so a longer gap is a question for your prescriber.
Changing your injection day
You can usually move your weekly day if you need to, as long as enough time passes between doses.
| Medication | Minimum time between two doses |
|---|---|
| Ozempic | At least 2 days (more than 48 hours) |
| Wegovy injection | At least 2 days |
| Mounjaro and Zepbound | At least 3 days (72 hours) |
Why a gap can affect side effects
These medications are started at a low dose and raised in steps. The Wegovy label says to restart escalation at a lower dose after several missed doses in order to reduce the risk of gastrointestinal side effects such as nausea. See the semaglutide and tirzepatide dosage charts for the standard schedules.
A practical note
- Set a recurring phone reminder for your injection day.
- Read the patient instructions that come with your medication, since they follow the current label.
- Tell your prescriber if you regularly miss doses, because a schedule change may help.
