Fatty Liver and Weight Loss: How Much Do You Need to Lose?

Last updated October 27, 2026

Short answer: The AASLD liver guidance says weight loss of 3% to 5% improves fat in the liver, but that more than 10% is generally required to improve the inflammation (NASH) and scarring (fibrosis) of fatty liver disease. A healthy diet and regular exercise are the foundation of treatment. Wegovy is also approved for noncirrhotic MASH with moderate to advanced scarring, based on a trial where people lost an average of 10.5%.

A note on the names

Fatty liver disease was long called NAFLD (nonalcoholic fatty liver disease), with its inflamed form called NASH. It is now more often called MASLD and MASH (metabolic dysfunction-associated steatotic liver disease and steatohepatitis). The terms mean closely related things, and the Wegovy label uses the newer ones.

How much weight loss is needed

GoalWeight loss the AASLD guidance describes
Less fat in the liver (steatosis)3% to 5% of body weight
Improve inflammation (NASH) and scarring (fibrosis)Generally more than 10%

The guidance also says a healthy diet and regular exercise are the foundation of treatment for the vast majority of people with fatty liver disease, that changes in diet composition appear comparable in their ability to improve it, and that the Mediterranean diet is often recommended. It says exercise should be routinely recommended and tailored to the person's preferences and physical abilities.

What the NIDDK suggests for eating

  • Gradual weight loss, if your doctor recommends it because you have overweight or obesity.
  • Limiting fats, and replacing saturated and trans fats with unsaturated fats, especially omega-3 fatty acids.
  • Eating more low-glycemic index foods.
  • Avoiding foods and drinks with large amounts of simple sugars, especially fructose.
  • Minimizing alcohol, which can further damage the liver.

Medication for MASH: what Wegovy showed

Wegovy injection is approved for noncirrhotic MASH with moderate to advanced liver fibrosis (stages F2 to F3) in adults. The label notes this approval is under accelerated approval and may depend on confirming the benefit in a further trial. In the 72-week analysis of 800 people with a liver biopsy, compared with placebo:

Biopsy result at 72 weeksWegovyPlacebo
Inflammation resolved, no worsening of fibrosis63%34%
Fibrosis improved, no worsening of inflammation37%22%
Both inflammation resolved and fibrosis improved33%16%

People on Wegovy in that trial lost an average of 10.5% of their weight, against 2% on placebo, which is in the range the guidance describes for improving inflammation and scarring.

What to do

  • Fatty liver is diagnosed by a clinician. If you think you may have it, ask about testing rather than self-treating.
  • Do not start or stop medicines for it on your own. Ask whether a weight management or MASH medicine fits your situation.
  • Use the percentage calculator to see what 5%, 7% and 10% would be at your weight.
